Zusammenfassung
Hypothalamische Hamartome sind die strukturelle Grundlage hochgradig pharmakoresistenter fokaler Epilepsien mit charakteristischen, im Kindesalter auftretenden emotionalen Anfällen und der späteren Entwicklung schwererer, propagierter Anfallstypen und einer resultierenden epileptischen Enzephalopathie mit kognitiver Verschlechterung und Verhaltensstörungen. Die Diagnose basiert auf der Konstellation von Anfallssemiologie und MRT-Befund. Bei Entwicklung stärkerer Anfallstypen stehen mittlerweile effektive minimal-invasive operative Therapieformen mit guter Verträglichkeit und Effektivität zur Verfügung. Unabhängig von der eingesetzten Behandlungsmethode führt eine Anfallskontrolle oder erhebliche Reduktion nichtemotionaler Anfallstypen in der Regel zu einer Rückbildung der epileptischen Enzephalopathie mit Verhaltensnormalisierung und verbesserter kognitiver Leistungsfähigkeit.
