Zusammenfassung
Epileptische Enzephalopathien (EE) sind Erkrankungen, bei denen epileptische Anfälle oder subklinische EEG-Anfallsmuster selbst zu Entwicklungsverzögerungen oder zum Verlust zuvor erworbener kognitiver Fähigkeiten beitragen. Zwei typische EE, die in der Kindheit beginnen, aber im Erwachsenenalter persistieren und somit auch für Neurologen relevant sind, sind das Dravet-Syndrom (DS) und das Lennox-Gastaut-Syndrom (LGS). DS und LGS zeigen Besonderheiten in Bezug auf Diagnostik und Therapie, auf die in diesem Kapitel ausführlich eingegangen werden soll. So gibt es Antiepileptika (Stiripentol, Felbamat, Rufinamid), die nur bei DS oder LGS zugelassen sind und mit denen Neurologen häufig wenig persönliche Erfahrungen haben. Außerdem soll die Wirksamkeit und Verträglichkeit von Cannabidiol diskutiert werden, das im Jahr 2019 für die Behandlung von DS und LGS in Zusatztherapie zu Clobazam zugelassen worden ist.
