Zusammenfassung
Genetische generalisierte Epilepsien umfassen eine Reihe von Epilepsiesyndromen, die maßgeblich durch das Alter bei Manifestation und den vorherrschenden Anfallstyp charakterisiert und oft auch benannt sind. Vier dieser Syndrome können bis in das Erwachsenalter persistieren; dies sind die kindliche und die juvenile Absence-Epilepsie, die juvenile myoklonische Epilepsie und die Epilepsie mit ausschließlich generalisierten tonisch-klonischen Anfällen. Genetische Ursachen sind anzunehmen, auch wenn diese in der Regel weder nachgewiesen werden können noch hinsichtlich ihrer pathophysiologischen Auswirkungen verstanden sind. Das Ansprechen auf Antiepileptika in Monotherapie ist grundsätzlich gut. Ethosuximid wirkt gut gegen Absencen, Valproinsäure gegen Myoklonien und generalisierte tonisch-klonische Anfälle. Die Prognose ist günstig; je älter Patienten werden, desto wahrscheinlicher sind sie anfallsfrei.
