Zusammenfassung

Ein 61-jähriger Patient wurde neurologisch vorgestellt, nachdem er unter 39,1 °C eine zunehmende Bewusstseinsstörung entwickelte. Der Patient war 12 Jahre zuvor nierentransplantiert worden und deshalb immunsupprimiert. Der Patient war kurz zuvor von einem Heimaturlaub in Montenegro zurückgekehrt. Im Liquor fanden sich 96 lymphozytäre Zellen/μl und eine Eiweißerhöhung auf 119 mg/dl. Kernspintomographisch zeigten sich hyperintense symmetrische Läsionen in der Substantia nigra und im Bereich des anterioren Zervikalmarks C3–C7. Die Verdachtsdiagnose einer West-Nil-Enzephalomyelitis wurde serologisch (ELISA) bestätigt.
