Zusammenfassung
Unter dem Verdacht auf ein Hirntodsyndrom wurde ein 33-jähriger tief komatöser Patient übernommen, der neben einem vollständigen Verlust der spinalen Reflexe auch Zeichen des Hirnstammfunktionsverlusts bot. Anhand des Leitbefundes „Hippus pupillae“ wurde über die Spike-wave-Muster im EEG und schließlich dem Nachweis von Strychnin im Urin ein Mischbild aus schwerer hypoxischer Hirnschädigung und Strychninintoxikation gesichert, der nach Eintritt des Hirntodes von ungewöhnlich lebhaften Spinalisationszeichen gekennzeichnet war. Das Leitsymptom „Hippus pupillae“ im Koma und der Einfluss von Strychnin auf die Spinalisation werden diskutiert.
