Zusammenfassung

Invasive Massnahmen sind bei der Diskusverlagerung, Diskus perforation und Arthrose wegen der besonderen Anpassungskapazität des Kiefergelenks nur selten erforderlich. Dagegen sind variable, erprobte Operationsverfahren bei einer Hypermobilitätstörung, chronischen Arthritis aus verschiedener Ursache, der kondylären Hyper- oder Hypoplasie, bei Erkrankungen, die zu freien Gelenkkörpern führen, bei Resorption des Kondylus, bei Ankylose und Tumoren oft ohne Alternative. Das Spektrum reicht von der Arthrozentese über arthroskopische Maßnahmen bis zur Arthrotomie mit standardisierten, abgestuften Verfahren bis zum autologen oder alloplastischen Gelenkersatz. Zugänge von prä-, retro- oder subauriculär bieten bei den verschiedenen Indikationen hohe Sicherheit bei kleinstmöglichem Gewebstrauma zur Vermeidung von Komplikationen und ungünstigen postoperativen Folgen.
