Zusammenfassung
Die OLIF-Technik hat sich in den 1990er-Jahren in Europa aus Zugangsvariationen der anterioren lumbalen interkorporellen Fusion entwickelt. Die Skalierbarkeit des Zugangs erlaubt Fusionseingriffe, Wirbelkörperersatz, Bandscheibenersatz sowie Revisionseingriffe. Analog zur ALIF-Technik (Anterior Lumbar Interbody Fusion) ist die Dekompression des Spinalkanals (wie an der HWS) möglich. Im Gegensatz zum ALIF wird der retroperitoneale Zugang durch Schwerkraftretraktion des Peritonealsacks erleichtert. Das anteriore longitudinale Ligament und der Anulus sind für ein vollständiges ventrales Release zugänglich. Wie beim ALIF steht eine Vielzahl von Implantatsystemen zur Behandlung degenerativer, traumatischer, neoplastischer und entzündlicher Erkrankungen der LWS zur Verfügung. In der Vorbereitung des Eingriffs hat sich die Durchführung einer CT-Angiografie bewährt. Intraoperative Assistenz durch einen Gefäßchirurgen ist zu Beginn der Lernkurve essenziell. Für die sichere und zeitökonomische Durchführung des OLIF sind ein geeignetes Retraktorsystem und Instrumente sowie eine gute Ausleuchtung des OP-Feldes erforderlich. Die Zugangsdimensionen limitieren die Rolle des chirurgischen Assistenten.
