Zusammenfassung

Bei Behandlung von Aneurysmen der deszendierenden Aorta sollte das endovaskuläre Vorgehen (TEVAR) dem offenen chirurgischen Eingriff vorgezogen werden, wenn die Anatomie dafür geeignet ist. Die Empfehlungen zu den thorakoabdominellen Aortenaneurysmen (TAAA) sind nicht so eindeutig, da in hochspezialisierten Zentren mit dem offenen Vorgehen hervorragende Ergebnisse erzielt werden – zumindest bei Patienten mit geringem oder mäßigem chirurgischem Risiko. Gleichwohl hat die technische Entwicklung von fenestrierten und gebranchten Stentgrafts dazu geführt, auch für das TAAA zunehmend das endovaskuläre Vorgehen oder Hybrideingriffe zu propagieren.
