Zusammenfassung
Das menschliche Endometrium ist für die Einnistung der Blastozyste nur in einem engen Zeitraum von 4–5 Tagen empfänglich. Der Prozess der Einnistung lässt sich in Apposition, Adhäsion und Invasion einteilen. Diese Prozesse werden von einem Netzwerk von zahllosen Faktoren gesteuert. Dazu zählen Hormone, vom Embryo sezernierte Faktoren, Leptin, Wachstumsfaktoren, Chemokine und hCG und endometriale Faktoren. Der Embryo wird über Muzine zum Implantationsort gelenkt. Nach dem Schlüpfen aus der Zona pellucida sezerniert der Embryo L-Selektin, das an Oligosaccharide auf der epithelialen Oberfläche bindet und so den Embryotransport verlangsamt, sodass eine Anheftung über Integrine und das Brückenprotein Osteopontin möglich wird. Anschließend beginnt die Invasion durch das maternale, luminale Epithel in das dezidualisierte Stroma hinein, u. a. gesteuert von Syndekanen, HB-EGF oder Galektin-9 und nach Anschluss an das mütterliche Gefäßbett die frühe Schwangerschaft.
