Zusammenfassung
Hauptvertreter der Osteoporosemittel sind Bisphosphonate, die in der Onkologie auch dem Schutz vor Knochenmetastasen dienen. Seit mehreren Jahren zeichnet sich ein Rückgang der Bisphosphonatverordnungen ab. Leitsubstanz der Bisphosphonate ist Alendronsäure, auf die jetzt knapp 70% des Verordnungsvolumens dieser Stoffgruppe entfallen, während Risedronsäure, Ibandronsäure und Zoledronsäure deutlich kleinere Anteile haben. Mit weitem Abstand folgt das weiterhin aufsteigende Denosumab, Strontiumranelat ist 2017 wegen sinkender Absatzzahlen und zahlreicher thrombembolischer und anderer Nebenwirkungen vom Markt genommen worden.
Calciumpräparate werden mit leichter Abnahme weiterhin als Basistherapeutika vor allem in Kombination mit Vitamin D eingesetzt, auch wenn sie nur einen bescheidenen Effekt auf die Frakturrate haben und vor allem bei Vitamin- D-Mangel wirksam sind. Weitere Calciumpräparate sind als Phosphatbinder zur Behandlung der Hyperphosphatämie bei Hämodialysepatienten von Bedeutung.
