Zusammenfassung
Die Zunahme der Adipositas insbesondere bei den jüngeren Erwachsenen stellt ein erhebliches gegenwärtiges und auch zukünftiges gesundheitspolitisches und sozioökonomisches Problem dar. Ein operativer Eingriff stellt in den meisten Fällen die gegenwärtig einzig verfügbare nachhaltige Therapieoption dar, die auch an erster Stelle stehen kann. Getrennt wird neuerdings zwischen der klassischen Adipositaschirurgie und der Metabolischen Chirurgie, bei letzterer wird die Indikation primär zur Verbesserung der glykämischen Stoffwechsellage bei einem vorbestehenden Typ-2-Diabetes gestellt. Das Therapieziel adipositaschirurgischer Eingriffe ist es, durch eine nachhaltige Gewichtsreduktion und metabolische Veränderungen Folgendes zu erreichen: Verbesserung der Lebensqualität, Remission, Besserung bzw. Prophylaxe von Begleiterkrankungen, Verlängerung des Überlebens und Erhalt der Teilhabe (am Arbeitsleben, am gemeinschaftlichen und kulturellen Leben).
