Zusammenfassung

Risiken und Nebenwirkungen sind ein vernachlässigtes Feld in Ausbildung, Praxis und Forschung der Psychotherapie. Dennoch ist inzwischen klar, dass es unerwünschte Effekte von Psychotherapien gibt. Diese betreffen sowohl Nebenwirkungen oder Erfolglosigkeit von angemessenen Therapien als auch Folgen von unprofessioneller Ausführung der Behandlung oder von Schädigung durch unethisches Verhalten. Zunächst wird eine Begriffsklärung vorgenommen und eine Systematik von Risiken und Nebenwirkungen, Misserfolgen und unethischem Verhalten vorgeschlagen. Neben historischen Fallbeispielen und Presseberichten werden empirische Studien dargestellt, nach denen Verschlechterungen bei etwa 10–12 % aller Patienten und ausbleibende Besserung bei weiteren 15–20 % der Patienten auftreten. Im Bereich unethischen Verhaltens liegen die meisten Daten zu sexuellen Übergriffen vor. Rund 6 % aller männlichen und 2 % aller weiblichen Therapeuten berichten von sexuellen Kontakten mit gegenwärtigen Patienten. Die übliche Ausbildung bietet keinen hinreichenden Schutz vor diesen Problemen. Daher müssen sowohl ethische Verfehlungen als auch Risiken und Nebenwirkungen explizit in Ausbildung, Intervision und Supervision sowie Forschung thematisiert werden.
