Zusammenfassung
Bei der Essstörung Anorexia nervosa kommt es durch strenge Kalorienreduktion zu einem gravierenden Gewichtsverlust mit einer Vielzahl somatischer Symptome und Komplikationen. Typisch für das Essverhalten bei Bulimia nervosa sind häufige, unkontrollierbare Essanfälle, denen meist selbstinduziertes Erbrechen folgt. Das Störungsmodell berücksichtigt prädisponierende Faktoren, auslösende Ereignisse und Faktoren der Aufrechterhaltung. Therapeutisch wird bei Essstörungen in 2 Phasen interveniert: Kurzfristig soll bei der Anorexia nervosa durch Gewichtssteigerung, bei der Bulimia durch eine Reduktion von Essanfällen und die Herstellung von normalem Essverhalten eine möglichst rasche Rückbildung der biologischen Dysfunktionen erreicht werden. Langfristig müssen zu Grunde liegende Problembereiche und für das Essverhalten bedeutsame funktionale Beziehungen sowie komorbide Störungen bearbeitet werden. Für die Anorexia nervosa wird ein nach operanten Prinzipien aufgebautes mehrstufiges Programm zur Gewichtssteigerung beschrieben. Für die Bulimia nervosa eignet sich ein Ernährungsmanagement, das weitgehend auf Kontrakten zum täglichen Essverhalten basiert.
