Zusammenfassung
Die Pseudarthroserate nach Klavikulaschaftfraktur variiert gravierend je nach Risikoprofil, Frakturform und Anzahl der Fragmente sowie nach dem gewählten Therapieverfahren. Das Risiko einer ausbleibenden Frakturheilung ist unter konservativer Therapie mit ca. 5–15 % im Vergleich zu den operativen Therapieverfahren erheblich höher; es steigt bei Dislokation der Knochenfragmente bzw. bei mehrfragmentären Frakturen noch weiter an. Bei den operativen Therapieverfahren beträgt das Risiko einer Pseudarthrose bei der Versorgung mittels Plattenosteosynthese ca. 5 % und bei der intramedullären Nagelung (ESIN) 1–3 %. Prinzip der Plattenosteosynthese bei der Klavikulaschaftpseudarthrose in Kombination mit einer autologen Beckenkammtransplantation ist es, die häufig komplexe Defektsituation stabil zu überbrücken, durch Kompression auf einen trikortikalen Span diesen stabil zu integrieren und das osteoinduktive Potenzial des vitalen Eigenknochens zu nutzen.
