Zusammenfassung

Wie bei allen Erkrankungen steht die Diagnostik selbstverständlich auch in der Proktologie am Anfang aller ärztlichen Behandlungsmaßnahmen. Die Schilderung der Beschwerden durch den Patienten ermöglicht fast immer bereits erste klare Schlussfolgerungen bezüglich des zugrundeliegenden Krankheitsbildes. In diesem Kapitel werden allgemeine Symptomatik und Untersuchungsmethoden der Proktologie abgehandelt. Neben den essenziellen Bereichen Anamnese, klinische Untersuchung und Basisdiagnostik existieren noch weitere Spezialuntersuchungen, die bei einzelnen Krankheitsbildern gezielt eingesetzt werden können, auf die man aber bei der Mehrzahl der Patienten gut verzichten kann. Dies gilt insbesondere für die kostenaufwendige bildgebende Diagnostik (Röntgen, CT, MRT). Ausführlich wird die Endosonografie behandelt, die als einfaches und wenig belastendes Verfahren Zusatzinformationen liefern kann. Der Abschnitt zur neurologischen Diagnostik wurde aus historischen Gründen unverändert in diese Ausgabe übernommen. In der täglichen Routine haben sich diese Untersuchungen meist nicht durchsetzen können. Der Abschnitt zur radiologischen und nuklearmedizinischen Diagnostik ergänzt aus Sicht der Bildgebung die Aussagen in den Spezialkapiteln zur Stuhlinkontinenz und Obstipation, in denen aus Sicht des Klinikers die Wertigkeit der Untersuchungstechniken für die Therapie beurteilt wird.
