Surgery of the adrenal gland has a fairly short history, as the understanding of the functional diseases of the adrenal required sophisticated biochemical techniques, and identification of small- to moderate-sized tumors was aided by the development of cross-sectional imaging techniques [1]. The adrenal glands are almost equidistant from the anterior surface, the lateral surface, and the posterior surface of the skin. In the era before laparoscopic surgery, therefore, adrenalectomy could be performed through a transabdominal approach, a flank approach, or a posterior approach [2] (Fig. 11.1).
