Zusammenfassung
Die symptomatische unkomplizierte Divertikelerkrankung (SUDD, Typ 3a nach der Klassifikation der Divertikelerkrankung) ist ein Krankheitsbild, dessen Pathophysiologie nicht komplett geklärt ist. Eine pathophysiologische Ähnlichkeit zwischen der Entstehung der Beschwerden des Reizdarms und denen der SUDD steht einer recht klaren klinischen Abgrenzbarkeit gegenüber. Die Therapieansätze ähneln denen der moderaten Entzündungsreaktion bei chronisch entzündlichen Darmerkrankungen. Eine recht gute Datenlage ist zur Behandlung mit Mesalazin zu verzeichnen. Dies veranlasste die DGVS zur Sk2-Leitlinienempfehlung von Mesalazin. Mittlerweile ist die Datenlage auch zum Rifaximin recht eindeutig. Probiotika können weiterhin nicht generell empfohlen werden, da hier die Datenlage heterogen ist. Zur Behandlung mit faserreicher Kost stehen bislang keine randomisierten Studien mit einer klaren positiven Aussage zur Verfügung.
