Thorakoskopisch-laparoskopische Ösophagusresektion

Zusammenfassung

Die thorakoskopisch-laparoskopische Ösophagusresektion ist ein etabliertes Verfahren und führt im Vergleich zur offenen Operation zu einer deutlichen Reduktion pulmonaler Komplikationen und einer Verbesserung des Kurzzeit-Outcomes. Die Radikalitätsprinzipien der onkologischen Ösophagektomie folgen beim minimalinvasiven Vorgehen denselben Regeln wie bei der offenen Resektion. Sowohl der abdominelle als auch der thorakale Teil kann minimalinvasiv erfolgen, wobei die thorakale Phase in Bauchlage eine besonders gute Übersicht über das Operationsfeld bietet. Operateur- und teambezogen sind fortgeschrittene Erfahrungen bei komplexen laparoskopischen Operationen ebenso empfehlenswert wie eine hohe Sicherheit bei konventionellen Ösophagektomien.
