Zusammenfassung
Die Achalasie ist eine seltene Funktionsstörung der Speiseröhre mit bisher ungeklärter Ätiopathogenese. Die Inzidenz liegt bei ca. 1–3 Neuerkrankungen/100.000 Einwohner pro Jahr. Die derzeit zur Verfügung stehenden Therapieoptionen zielen auf eine Verbesserung der Nahrungspassage über den ösophagogastralen Übergang sowie die Prävention der Progression zum Megaösophagus. Wegweisend für mögliche Therapiealgorithmen ist – neben dem Alter des Patienten mit besserem Ansprechen jüngerer Patienten auf die laparoskopische Kardiomyotomie nach Heller in Kombination mit einer Antirefluxplastik – die präoperative Diagnostik mittels High-Resolution-Manometrie (HRM). Gemäß der aktuellen Chicago-Klassifikation existieren 3 Achalasie-Subtypen. Insbesondere Patienten mit Typ-I- und Typ-II-Achalasie sprechen sehr gut auf die laparoskopische Kardiomyotomie an. Die Typ-III-Achalasie (spastische Achalasie) hingegen stellt eine besondere therapeutische Herausforderung dar: Hier ist eine lange Myotomie indiziert (perorale endoskopische Myotomie [POEM] oder laparoskopische Myotomie), die das gesamt spastische Segment des distalen Ösophagus umfasst. Die HRM-basierte Myotomie bietet aufgrund ihrer individuell angepassten Länge die optimale Voraussetzung für eine maßgeschneiderte Therapie der Achalasie.
