Zusammenfassung
Dysphagiebeschwerden treten im Alter gehäuft auf, vor allem bei pflegebedürftigen Patienten. Anamnese und klinische Untersuchung geben häufig schon wichtige Hinweise auf die Ursache (v. a. neurogen, pharyngeal, medikamentös, psychogen). Bei unklaren oder „Alarmsymptomen“ (wie progrediente/rezidivierende Dysphagie, GI-Blutung, Gewichtsabnahme, rezidivierendes Erbrechen, rezidivierende Aspirationspneumonie) sollte früh eine hochauflösende Videoendoskopie des oberen Verdauungstraktes (ÖGD) mit Biopsie erfolgen. Findet sich dabei ein maligner Prozess, sollte man weiter unbedingt nach den neuen Leitlinien der DGVS vorgehen und ein weiteres strukturiertes Tumor-Staging durchführen. Dazu gehören bildgebende Verfahren (v. a. Sonografie, CT, PET) und die lokoregionäre Endosonografie (EUS), die zusammen eine hohe Treffsicherheit für eine präoperative, klinische Stadieneinteilung der Ösophaguskarzinome erlauben, von dem das weitere, differenzierte, therapeutische Vorgehen entscheidend abhängt.
