Thoracoscopic Repair of Pectus Carinatum: Complete Cartilage Resection with Perichondrial Preservation Technique

Abstract

Thoracoscopic treatment is one of the options for the repair of Pectus Carinatum (PC). The ideal indication is a unilateral PC involving cartilages only, without sternal deformity or rotation. Very severe degree of PC represents a contraindication to thoracoscopic repair.

There are two similar techniques differing in some details. One of them proposed by Kim and Idowu in 2009 consists in removing 5 mm segments of bone and cartilage in multiple rows, in order to optimally affect the depression of the chest wall. Varela and Torre in 2010 proposed a modification of this technique applying the principles of the open surgical techniques that provide the complete cartilage resection of the abnormal ribs, preserving the anterior perichondrium.

Both techniques are performed with three thoracosocpic trocars inserted in the posterolateral thoracic wall.

The thoracoscopic treatment of PC gives good results if the indications are respected. It can be associated with other treatments as Abramson procedure or dynamic compression system.
