Zusammenfassung
Beim Transport des Patienten auf die Intensivstation darf keine diagnostische und therapeutische Lücke entstehen. Die Übergabe an das Team der Intensivstation sollte standardisiert, strukturiert und nach klinikinternem Protokoll erfolgen. Die postoperative Überwachung der ersten Stunden ist personell und apparativ aufwendig und sollte ebenfalls straff strukturiert sein. Bei hämodynamischer Instabilität gehört v. a. die Echokardiographie zum erweiterten hämodynamischen Monitoring. Die Zielvariablen der postoperativen Herz-Kreislauf-Therapie sind in der S3-Leitlinie der DGAI/DGTH definiert worden. Im Vordergrund stehen die Stabilisierung von Herzfrequenz und -rhythmus, die Optimierung der Vorlast des Herzens und die Unterstützung der Myokardkontraktilität sowie die Senkung der Nachlast bei schwer geschädigtem Myokard. Postoperative Nachblutungen sind eine allgegenwärtige Gefahr. Die meisten Patienten werden für etwa 6–24 h nachbeatmet und können dann extubiert werden.
