Zusammenfassung
Die Anschlussmöglichkeit des dialysepflichtigen Kindes an die künstliche Niere wird wie im Erwachsenenalter durch die verfügbaren potentiellen Shuntgefäße begrenzt. Das zur Dialysebehandlung verwendete Gefäßsystem muss möglichst einfach zu handhaben sein und pro Zeiteinheit eine ausreichende Blutmenge zur Dialyse verfügbar machen. Ellenbeuge und Axillarbereich bieten meistens die zum Anschluss ausreichend weitkalibrigen Gefäße. Ausnahmsweise sind bei dieser Klientel im Fuß oder auf Leistenhöhe geeignete Gefäße zum Shuntanschluss zu finden. Sofern aufgrund der aktuellen Behandlungsstatistik davon ausgegangen werden kann, dass das dialysepflichtige Kind innerhalb einer überschaubaren Zeitspanne von wenigen Monaten zur Transplantation gebracht werden kann, sollte man von vornherein den Zentralvenenverweilkatheter zur Dialysebehandlung nutzen und auf die eigentliche Shuntanlage verzichten.
