Supermikrochirurgie der Gefäße und Nerven

Zusammenfassung

Supermikrochirurgie bezeichnet die Technik der Anastomosen kleiner Gefäße oder Koaptation einzelner Nervfaszikel und deren Präparation. Der Gefäßdurchmesser für die Supermikrochirurgie ist definiert <0,8 mm. Zu den Prinzipien gehören gewebeschonendes Operieren, das Zug- und Scherkräfte am Gefäß vermeidet, kurze Gefäßstiele, individualisierte Lappenplastiken und Anschlüsse an Perforansgefäße. Aufgrund der Anatomie solcher kurzen Perforansgefäße, die häufig sehr dünnwandige Arterien und Venen haben, benötigt man eine spezielle Anastomosentechnik, spezielles Instrumentarium und ein lichtstarkes Mikroskop. Zur Anastomose sollte ein adäquater Flow im Gefäß herrschen. Grundsätzlich können fast alle bekannten Lappenplastiken als supermikrochirurgische Lappenplastiken gehoben und transplantiert werden. Andere Lappenplastiken werden aber für supermikrochirurgische Indikationen neu beschrieben werden, wenn einfach die Perforansgefäße »free style« gehoben werden oder bisher als zu klein erachtete Gefäßstiele für Muskeln als transplantierbar eingestuft werden.
