Conservative Treatment of Talar Osteochondritis Dissecans (OCD)

Abstract

The main goals in the treatment of talar osteochondritis dissecans (OCD) are to regenerate articular cartilage, thereby reducing pain and improving function. Most of the treatments include operative procedures. However, there are no widely accepted protocols for conservative treatment. Conservative treatment today is, in the main, rest and immobilization. Sometimes, physiotherapy can improve function of the limb. Injections to the joint traditionally include steroids, but there are no papers concerning treatment of talar OCD with steroid injections, and steroids are usually short acting. Injections of viscosupplementation – hyaluronic acid and platelet rich in growth factor (PRGF) – are suggested both as an alternative to the treatment of osteoarthritis of the ankle and may relieve pain and improve function in talar OCD which is localized articular damage. Treatment with hyperbaric oxygenation may reduce bone marrow edema in patients with talar OCD. However, more research is needed to evaluate these modes of treatment and their efficacy.
