Zusammenfassung
Die häufigste Fehlstellung nach operativer oder konservativer Therapie von Sprunggelenkfrakturen ist die Verkürzung der distalen Fibula. Sie ist häufig kombiniert mit einer Fehlrotation, Achsenabweichung und einer Fehlstellung derselben in der Tibiainzisur bei nicht adäquat behandelter Syndesmosenverletzung. Seltener finden sich relevante, korrekturbedürftige Fehlstellungen des Innenknöchels und der Tibiahinterkante. Das Prinzip der Korrekturosteotomie der distalen Fibula besteht in der Rekonstruktion von Achse, Länge und Rotation sowie in der korrekten Einstellung derselben in die korrespondierende Inzisur der Tibia bei chronischer Syndesmoseninsuffizienz. Ziel ist die Wiederherstellung der Gelenkkongruenz unter Erhalt bzw. Verbesserung der Funktion sowie die Verhinderung der Entstehung oder Progredienz einer posttraumatischen Arthrose im oberen Sprunggelenk.
