Zusammenfassung
Der artifizielle Sphinkter wurde 1972 von Bradley und Scott entwickelt und von AMS fortentwickelt. Das Sphinktersystem besteht aus einer um die Harnröhre platzierten Manschette, einem intraperitoneal gelegenen Ballonreservoir und einer Kontrollpumpe, die in die rechte große Labie implantiert wird. Der Sphinkter wird unter Kontrolle des Blasenhalses von suprapubisch implantiert. Eine Eröffnung der Scheide sollte während der Operation unbedingt vermieden werden, da dadurch eine bakterielle Kontamination des gesamten Systems erfolgt. Der Druckgradient des Ballons beträgt 61–70 cmH2O, die Reservoirfüllung 22 ml und die Cuff-Weite meist 6,0 cm (5–7 cm). Bei hochsteriler Implantationstechnik, guter intraoperativer Blutstillung und Beherrschung der Operationstechnik ist die Komplikationsrate relativ gering.
