Summary

Knowledge of the pathophysiological bases of drowning is the prerequisite for correctly applied first aid. The entry of water into the respiratory tract most of all differentiates drowning from pure suffocation. Selective therapeutic measures are required on account of the entry of fresh water into the circulation as well as on account of foam formation in the alveoli and small bronchi which prevents gas exchange. Spontaneous pneumothorax is a frequent complication after survival of a drowing accident. Thus clinical observation for several days is always indicated.

Zusammenfassung

Die Kenntnis der pathophysiologischen Grundlagen des Ertrinkungsunfalls ist Voraussetzung für eine richtig angewandte Erste Hilfe. Vor allem das Eindringen von Wasser in die Luftwege unterscheidet das Ertrinken vom reinen Ersticken. Durch den Übertritt von Süßwasser in die Blutbahn sowie durch die den Gasaustausch verhindernde Schaumbildung in den Alveolen und kleinen Bronchien sind gezielte therapeutische Maßnahmen erforderlich. Eine häufige Komplikation nach Überleben eines Ertrinkimgsunfalles ist der Spontan-pneumothorax. Mehrtägige klinische Beobachtung ist daher immer angezeigt.
