Zusammenfassung
In Bezug auf Häufigkeit und Schwere des Krankheitsbildes stellt das Dengue-Fieber die wichtigste tropische Virusinfektion dar. Pro Jahr rechnet man mit mit mehreren Millionen Infektionen insgesamt. Auch mehrere Tausend deutsche Touristen erkranken schätzungsweise. Durch die Zunahme von Insekten und durch den Flugverkehr haben sich die vier Dengue-Stämme weltweit stark ausgebreitet. Es besteht die Möglichkeit, das Dengue-Fieber mehrfach und zwar mit zunehmender Schwere zu bekommen. Zweitinfektionen, die inzwischen auch bei Touristen gefunden werden, können besonders bei Kindern lebensbedrohlich sein, wenn sich ein hämorrhagisches Fieber entwickelt. Eine primäre Dengueinfektion kann durch Bestimmung von IgM- und IgG-Antikörpern gegen das Virus festgestellt werden. Für die Bestimmung des für die Infektion verantwortlichen Serotypes ist die PCR geeignet. Differentialdiagnostisch ist an eine Reihe von anderen Parasiten- und Virusinfektionen zu denken. Die Therapie ist symptomatisch. Infusionen können bei schwerem Dengue-Fieber lebensrettend sein.
Summary
Considering its frequency and fatality rate dengue fever repesents the most important tropical virus infection. Several millions of infected people including thousands of German tourists are recorded annually. Due to the increase of insect populations and intercontinental travelling the four dengue serotypes nowadays can be found worldwide. Sequential dengue fever infections have been reported to occur with increasing severity. Secondary infections, which are also found in tourists, may be associated with hemorrhagic fever and may be life- threatening, especially in children. During primary infection, a laboratory diagnosis can be made by detecting both IgM- and IgG-antibodies. The serotype involved can be identified by PCR. Dengue fever-like symptoms can also be caused by a variety of parasitic or virus infections. Although a specific therapy is not available, intravenous infusions may be of vital importance during dengue hemorrhagic fever.
