Zusammenfassung
Kreislaufsymptome wie Tachykardie, Herzklopfen, Atemnot bei Anstrengungen spielen eine Hauptrolle in der Symptomatologie der Thyreotoxikosen. Sie wurden schon von den ersten Krankheitsbildes, so von Parry (1825) und Basedow (1840), in ihrer Bedeutung erkannt. Durch die erhebliche Steigerung des Herzminutenvolumens sowie die Tachykardie wird den Kreislauforganen schon in der Ruhe eine erhebliche und andauernde Mehrbelastung zugemutet, welche die Leistungsreserve bei zusätzlichen Belastungen — etwa durch Arbeit — einschränkt. Die dauernd überbeanspruchten Kreislauforgane können organische Kreislaufschäden, die sich im Laufe des Lebens, etwa als Folge von Infekten oder durch die Entwicklung einer Arteriosklerose einstellen, weniger gut kompensieren. Und so wird der Basedowiker, soweit er nicht geheilt wird oder an interkurrenten Krankheiten stirbt, nahezu zwangsläufig zum Herzkranken. Das Auftreten einer Arrhythmia absoluta, einer Herzdilatation, sind oft die Vorboten einer sich entwickelnden Herzinsuffizienz mit Stauung. Die Einschränkung der „Coronar-Reserve“ begünstigt das Auftreten einer Angina pectoris. Es ist daher müßig zu fragen, ob die Hyperthyreose allein einen spezifischen ursächlichen Typ von Herzkrankheit darstellt oder ob sie nur im Zusammenwirken mit anderen organischen Schäden, die sie gewissermaßen demaskiert (Spang u. Korth 1939), das Herz zum Versagen bringt.
