Zusammenfassung
Viele Tumorpatienten erhalten zu irgendeinem Zeitpunkt ihrer Erkrankung eine Strahlenbehandlung und eine zytostatische Chemotherapie. Manchmal ist die Gabe beider therapeutischer Modalitäten geplant und aufeinander abgestimmt, in den meisten Fällen wird aber die eine oder andere Behandlungsart eingesetzt, um eine Tumorprogredienz zu bekämpfen. Bei diesen primär nicht geplanten Kombinationen ist es wichtig, die Nebenwirkungen der Strahlen- und Chemotherapie auf bestimmte Organe oder Organsysteme zu betrachten und bei der Therapieplanung zu berücksichtigen. Viele Spätfolgen sind in einem subklinischen Stadium und werden erst manifest, wenn sich gleiche oder ähnliche Noxen addieren. Dies kann an 2 Beispielen demonstriert werden:

Zunächst wird die Behandlung eines Plattenepithelkarzinoms mit Bleomycin durchgeführt, ohne daß unerwünschte bleibende Spätfolgen bemerkt werden. Zu einem späteren Zeitpunkt wird es erforderlich, eine palliative Strahlentherapie von parailiakalen Lymphknotenmetastasen durchzuführen. Das Zusammenwirken der Effekte am Bindegewebe führt im Anschluß an die Stahlenbehandlung — typischerweise 6–9 Monate später — zur Ausbildung einer Fibrose mit Ummauerung der großen Gefäße und nachfolgender Abflußbehinderung des venösen Blutes aus der unteren Extremität, Ausbildung eines Lymphödems des Beines und einer Beckenvenenthrombose.

Zunächst wird eine Strahlentherapie des oberen Abdomens durchgeführt, bei der die Nieren eine Dosis erhalten, die innerhalb ihrer Strahlentoleranz liegt. Im späteren Verlauf der Tumorerkrankung wird die Chemotherapie von Metastasen mit Cisplatin erforderlich, das in hohem Maße nephrotoxisch ist. Vor der Gabe von Cisplatin ist die Nierenfunktion nicht nachweisbar eingeschränkt, die zusätzliche Noxe führt jedoch schon bei einer Dosis des Chemotherapeutikums, die unerwartet niedrig ist, zur Niereninsuffizienz.
