Zusammenfassung

Bei der Behandlung des Ösophaguskarzinoms bekommt die Chirurgie wieder zunehmend Bedeutung. Dies besonders in Frühstadien sowie in Kombination mit der Strahlen- und/oder Chemotherapie; die Chemotherapie wird dabei auch in der präoperativen, der sog. neoadjuvanten Form appliziert, besonders in lokal fortgeschrittenen, irresektablen Stadien. Chirurgische Tumorentfernung ist wohl auch bei Malignomen dieses Organs am wirkungsvollsten hinsichtlich erreichbarer Lebensverlängerung, gelegentlicher Tumorheilung und effektiver Palliation. Eine deutliche Verringerung der Operationsletalität, besonders durch weitgehende Vermeidung von Anastomoseninsuffizienzen, hat zu der Indikationsausweitung zum resezierenden chirurgischen Vorgehen wesentlich beigetragen. Dabei wurde auch eine subtile mediastinale Lymphadenektomie eingeführt. Dagegen haben bei irresektablen Tumoren endoskopische Verfahren (Lasertherapie, endoskopische Tubusimplantation) höhere Bedeutung als große palliative Umgehungsoperationen.
