Psychopharmakotherapie bei somatischen Erkrankungen

Zusammenfassung

Eine Indikationsstellung für Psychopharmaka bei somatisch komorbiden Patienten ist in der Regel nur nach enger Abstimmung mit den behandelnden Ärzten der jeweiligen somatischen Fachgebiete zu empfehlen. Grundsätzlich kann jedes psychiatrische Syndrom durch eine bereits vorhandene körperliche Grunderkrankung bedingt bzw. als Folge von pharmakokinetischen und pharmakodynamischen Wirkungen (einschließlich Überdosierungserscheinungen) einer vorbestehenden Medikation angesehen werden. Insofern empfiehlt sich für den Konsiliarpsychiater auch die Erwägung von möglichen pharmakologischen Wirkungen, z. B. von internistischen Medikamenten. Eine Psychopharmakotherapie nach einem Konsil wird meist syndromgeleitet sein, z. B. in der symptomatischen Behandlung eines deliranten Zustandes, von psychomotorischer Agitiertheit oder als antidepressive Pharmakotherapie bei einem Patienten mit schweren karzinombedingten Schmerzen. Zum anderen trifft der Konsiliarpsychiater auch auf Patienten mit psychiatrischer Grunderkrankung, die zusätzlich eine somatische Krankheit entwickeln. Bei diesen Patienten stellt sich die Frage nach der Zweckmäßigkeit der Weiterführung einer Psychopharmakotherapie und deren Kombinierbarkeit mit einer somatischen Behandlung, z. B. auf einer Intensivstation.
