Zusammenfassung

Angeborene Herzfehler manifestieren sich mit den Zeichen der Herzinsuffizienz, Zyanose und Herzgeräuschen, sehr selten mit Synkopen oder mit plötzlichem Herztod ohne warnende Hinweise. Herzthoraxschmerzen, vor allem unter Belastung, können Symptome einer Kardiomyopathie, einer Koronargefäßfehlbildung oder einer Krankheit des Perikards sein. Weitaus häufiger haben sie extrakardiale Ursachen, wobei psychosomatische Vorgänge in der Häufigkeit dominieren. Eine charakteristische, tageszeitliche Änderung des Aktivitätsrhythmus („Morgenmuffel“) mit morgendlicher Übelkeit, Frühstücksverweigerung, erheblichen Anlaufschwierigkeiten, abendlicher Leistungszunahme („Abendaktivisten“ mit großer Ausdauer) sowie der Situation unangemessen hohe Herzfrequenzen, Leistungsschwäche, Ohnmachtsanfälle weisen auf Kreislaufregulationsstörungen hin.
