Zusammenfassung
Im Knochen können sich granulomatöse Prozesse entwickeln, die oft klinisch und röntgenologisch große diagnostische Schwierigkeiten bereiten. Allen granulomatösen Läsionen gemeinsam ist ihre primäre Entstehung im Markraum eines Knochens, von wo aus sich ein solcher Herd in die Kortikalis ausdehnt, indem er zuerst eine Arrosion der endostalen Seite der Kortikalis hervorruft, was im Röntgenbild zur Darstellung kommt. Meistens handelt es sich um eine lokale osteolytische Läsion, da im Bereich des Granuloms die Spongiosabälkchen zerstört werden. Dadurch entsteht das Bild einer „Knochenzyste“. Im Röntgenbild stellen die Randreaktionen im Bereich eines solchen Osteolyseherdes ein sehr wichtiges diagnostisches Kriterium dar. In selteneren Fällen kann auch eine umschriebene Osteolyse in der Kortikalis von einem Knochengranulom stammen.
