Zusammenfassung
Der großen Zahl von Fällen infektiöser Endokarditis durch Gram-positive Erreger steht erstaunlicherweise eine geringe Zahl Gram-negativer Fälle gegenüber — erstaunlich, weil Bakteriämien mit Gram-negativen Keimen nicht selten sind, häufig genug induziert durch diagnostische oder therapeutische Eingriffe im gastro-intestinalen oder urogenitalen Bereich, besonders bei älteren Personen mit prädisponierenden Herzklappenveränderungen. Auch mehren sich Gram-negative Endokarditiden bei Patienten mit Klappenprothesen oder anderen intravaskulären oder intrakardialen „Fremdkörpern“ und bei Drogensüchtigen. Dennoch machen sie trotz einer gewissen Häufigkeitszunahme nur einen kleinen Teil der bakteriellen, der infektiösen Endokarditis aus. So werden Zahlen zwischen 1,3 und 4,8% der Fälle angegeben (Scheid & Sande 1979); Cohen et al. (1980) geben 2,4% an. In Übersichten mit einer größeren Inzidenz von Drogen-induzierter Endokarditis oder unter Einschluß zahlreicher Patienten nach Klappenersatz finden sich Gram-negative Infektionen bis >10% (Finland & Barnes 1970; Karchmer & Swartz 1977; Kloster 1978; Delgado & Cobbs 1979; Watanakunakorn 1979). Tabelle 1 gibt die prozentuale Verteilung Gram-negativer Erreger unter 348 Patienten mit infektiöser Endokarditis wieder (Cohen et al. 1980). Als Erklärungen für die relative Seltenheit werden folgende Phänomene angeführt:
