Zusammenfassung
Der plötzliche Herztod (PHT) ist eine der großen diagnostischen und therapeutischen Herausforderungen unserer Tage. Alleine in Deutschland fordert er jährlich schätzungsweise 100 000 bis 140 000 Menschenleben, was größenordnungsmäßig einer Stadt wie Erlangen entspricht. Erfolgreich reanimierte PHT-Patienten sind besonders rezidiv gefährdet. Von den verschiedenen Therapiemodalitäten, die im Kampf gegen den plötzlichen Herztod entwickelt wurden, hat vor allem die medikamentöse Therapie enttäuscht (11). Nach den Ergebnissen der CAST- und ESVEM-Studien erscheint allenfalls noch der Einsatz von Klasse-III-Substanzen gerechtfertigt (9, 29). Von den ablativen Verfahren hat in bezug auf die Suppression ventrikulärer Tachyarrhythmien die Kathetherablation derzeit noch experimentellen Charakter und ist nur in wenigen speziellen Fällen bei einer ausreichend niedrigen Rezidivrate erfolgversprechend (3, 28). Die chirurgischen Ablations verfahren hingegen haben bei bestimmten Indikationen durchaus ihre Berechtigung, sind allerdings durch eine relativ hohe Operationsletalität von 7–12% belastet (28). Demgegenüber rückt der implantierbare Kardioverter-Defibrillator (ICD) aufgrund seiner konkurrenzlos hohen Effektivität den PHT zu verhindern, zunehmend ins Zentrum des klinischen Interesses.
