Zusammenfassung

Die Insulinsubstitution wird durch einen individuell sehr unterschiedlich ausgeprägten Zirkadianrhythmus der Insulinwirkung und damit des Insulinbedarfs beeinflußt. Die häufige Morgenhyperglykämie (Dawn-Phänomen) und die Nachmittagshyperglykämie (Dusk-Phänomen) erschweren die Stoffwechseleinstellung. Die posthypoglykämische Hyperglykämie (Somogyi-Phänomen) wird in ihrer Häufigkeit und Bedeutung meist überschätzt. Die Nebenwirkungen Insulinallergie, Insulinresistenz und Lipodystrophie sind durch die Verwendung hochgereinigter Insulinpräparationen sehr selten geworden.
