Zusammenfassung
Mittlerweile steht eine Vielzahl von Methoden zur Untersuchung von thrombozytären Antigenen und Antikörpern zur Verfügung. Plättchen thrombozytopenischer Patienten werden zur serologischen Bestimmung der Alloantigene und zur Quantifizierung zellständiger thrombozytärer Autoantikörper isoliert. Eine Typisierung der thrombozytären Alloantigene ist bei extrem thrombozytopenischen Patienten durch Analyse der genomischen DNS möglich. Diese kann aus beliebigen kernhaltigen Zellen gewonnen werden. Thrombozyten gesunder Blutspender finden darüber hinaus in Thrombozytenpanels zur Charakterisierung thrombozytenreaktiver Antikörper Verwendung. Freie plättchenreaktive Antikörper in Patientenseren werden mit Hilfe des Enzymimmunoassays für thrombozytäre Antikörper oder des Plättchenadhäsions-Immunfluoreszenztests nachgewiesen. Die Identifizierung der Proteinspezifität thrombozytärer Antikörper gelingt meist mit Hilfe der Immunpräzipitation oder technisch einfacher mit Hilfe des MAIPA-Assays. Bei Patienten mit erhöhten plättchenständigen Immunglobulinen ist eine Autoimmunthrombozytopenie wahrscheinlich, wenn sich diese auf den Glykoproteinkomplexen II b/III a oder I b/IX lokalisieren lassen (direkter MAIPA) oder wenn ein Säureeluat der autologen Thrombozyten einen plättchenreaktiven Antikörper enthält, der in einem Bindungstest wie dem Immunfluoreszenztest nachgewiesen werden kann.
