Zusammenfassung
In den vergangenen Jahren wurden die Grenzen der Serumbilirubinwerte bei Neugeborenen immer wieder in Frage gestellt,da das Auftreten eines Kernikterus,dessen Verhinderung der Hintergrund einer jeden Therapie ist,zur Seltenheit wurde. Im Gegensatz dazu versuchen neuere Studien zu belegen,dass auch moderat erhöhte Bilirubinwerte bei gesunden Neugeborenen mit einem Risiko der statomotorischen Beeinträchtigung assoziiert sind. Durch kürzere Hospitalisierungszeiten der Neugeborenen und ihrer Mütter kommt es außerdem immer wieder vor, dass ansonsten gesunde Neugeborene exzessiv hohe Bilirubinwerte im Serum aufweisen. Das Problem der Therapieindikation bei Hyperbilirubinämie besteht aber darin,dass randomisierte, kontrollierte Studien zu sicheren Grenzwerten, unterhalb derer nicht therapiert werden muss, fehlen. Die hier vorgestellten Therapieansätze basieren demnach auf den Konsensusempfehlungen nationaler und internationaler Fachgesellschaften. Sie stellen jedoch nur einen Anhalt dar,Therapieentscheidungen müssen immer individuell für den jeweiligen Patienten unter Einbeziehung aller Kofaktoren, die einen Kernikterus begünstigen, getroffen werden.
