Zusammenfassung

Als Ileus wird eine Transportstörung des gesamten Darmes oder von Darmabschnitten bezeichnet, die in über 80 % durch ein mechanisches Hindernis verursacht wird und nur selten aufgrund einer funktionellen Motilitätsstörung der Darmmotorik entsteht. Die Einteilung der Ileusformen erfolgt nach klinischen Gesichtspunkten, wobei zwischen akuten, subakuten und chronischen Verlaufsformen sowie einer kompletten oder inkompletten Passagestörung unterschieden wird. Hinsichtlich ihrer Lokalisation sind außerdem der hohe und tiefe Dünndarmileus vom Dickdarmileus und gemischten Ileusformen abzugrenzen. Die bei der Passagestörung entstehende Stauung des Darminhalts führt über verschiedene pathophysiologische Mechanismen zur so genannten „Ileuskrankheit“, die unbehandelt im Multiorganversagen mit letalem Ausgang endet.
