Zusammenfassung
Der natürliche Verlauf der chronischen Niereninsuffizienz ist progredient Die Progredienz wird unabhängig von der Grunderkrankung von sekundären Faktoren beeinflusst Als wichtigste Progressionsfaktoren betrachtet man die intraglomeruläre Hypertonie und die glomeruläre Hypertrophie. Diese sind für die adaptive Hyperfiltration und später für die Glomerulosklerose verantwortlich. Einfluss auf die Geschwindigkeit des Nierenfunktionsverlustes haben außerdem intraglomeruläre Lipid- und Kalziumphosphatablagerungen, tubulointerstitielle und hyperglykämiebe-dingte Schäden und die metabolische Azidose.
Untersuchungen beim Menschen haben ergeben, dass ACE-Hemmer-Therapie die Progression der chronischen Niereninsuffizienz sowohl bei diabetischer Nephropathie als auch bei nichtdiabetischer Niereninsuffizienz blutdruckunabhängig verlangsamt Auch normotensive Patienten mit Proteinurie sollten mit ACE-Hemmern zumindest niedrigdosiert behandelt werden, soweit der darunter auftretende Blutdruckabfall toleriert werden kann. Es ist zu vermuten, dass ein gleicher Effekt mit AT1Rezeptorantagonis-ten zu erzielen ist. Die Blutdruckeinstellung sollte bei Niereninsuffizienz niedrig normale Blutdruckwerte anstreben. Es sollten primär solche Medikamente Einsatz finden, die eine Proteinurie signifikant vermindern können, also ACE-Hemmer und AT1-Re-zeptorantagonisten. Die Kombination mit nicht peripher wirkenden Kalziumantagonisten ist günstig. Eine diätetische Eiweißrestriktion auf 0,8 g/kgKG/24 h mindert die Progredienz einer chronischen Niereninsuffizienz bei fast allen Grunderkrankungen.
Die Behandlung von Hyperlipidämie, metabolischer Azidose (besonders im Wachstumsalter), Hyperphosphatämie und Hyper-homozysteinämie ist ebenfalls zu empfehlen. Eine Trinkmenge im oberen Normbereich und darüber gleicht die mit zunehmender Niereninsuffizienz sinkende Harnkonzentrationsfahigkeit aus. Die Trinkmenge muss sich jedoch auch an der kardialen Belastbarkeit orientieren. Auch therapieresistente Ödeme (z.B. beim nephrotischen Syndrom) können einen hohen Flüssigkeitsdurchsatz verbieten. Diese genannten Maßnahmen sollten bereits bei geringgradiger Niereninsuffizienz zum Einsatz kommen. Bei fortgeschrittener Niereninsuffizienz und Gefahr der Malnutrition muss die Eiweißrestriktion rechtzeitig beendet werden.
