Zusammenfassung
Integraler Bestandteil einer perioperativen Patientenversorgung sind intraoperative Schmerzausschaltung und postoperative Schmerzbehandlung. Grundsätzlich stehen Allgemeinanästhesie und Regionalanästhesieverfahren, sowie deren Kombination für die intraoperative Schmerzausschaltung zur Verfügung. Bei den Regionalanästhesieverfahren sind Rückenmark oder peripherer Nerv Zielstrukturen einer topischen Wirkung von Lokalanästhetika. Eine Allgemeinanästhesie bewirkt durch Hypnose, Analgesie und Muskelrelaxation die reversible Ausschaltung von Bewußtsein, Schmerz und Muskelbewegungen. Heute allgemeinübliche Narkosetechniken erzielen durch Kombination von zum Teil spezifisch, zum Teil unspezifisch wirksamen Substanzen diese einzelnen Komponenten einer Allgemeinanästhesie, nahezu unabhängig voneinander mit einem Minimum an unerwünschten Nebenwirkungen. So ist eine intraoperative Schmerzausschaltung, die nach den vegetativen Zeichen unzureichender Analgesie wie Blutdruck- und Herzfrequenzanstiegen, Pupillenreaktion, Schweißsekretion usw. gesteuert wird, fast immer problemlos zu erzielen.
