Zusammenfassung
Patienten befinden sich bei thoraxchirurgischen Eingriffen häufig in Seitenlage unter Allgemeinanästhesie und Lähmung der quergestreiften Muskulatur, und die Brustwand ist eröffnet. Verglichen mit dem Wachzustand und der aufrechten Körperhaltung können alle 4 Aspekte (Seitenlage, Anästhesie, Relaxation und Eröffnung der Pleurahöhle) zu erheblichen Veränderungen der Verteilung von Belüftung und Durchblutung in der Lunge führen. Zusätzlich wird ein Großteil der Lungenoperationen an der stillstehenden oder kollabierten, in Seitenlage oben liegenden Lunge (obere Lunge: OL) durchgeführt, während die in Seitenlage unten liegende Lunge (untere Lunge: UL) weiterhin belüftet wird und während der Zeit der Einlungenbeatmung ausschließlich oder ganz überwiegend den pulmonalen Gaswechsel bewerkstelligen muß. Die Einlungenventilation (ELV) bringt gegenüber der Zweilungenventilation (ZLV) weitere tiefgreifende Veränderungen des Belüftungs-Durchblutungs-(VA/ Q-)Verhältnisses mit sich.
