Zusammenfassung
Ein Truncus arteriosus liegt vor, wenn nur ein großes Gefäß aus der Herzbasis entspringt, das den System-, den Pulmonal- und den Koronarkreislauf versorgt. Dabei muß mindestens eine Pulmonalarterie direkt stromab der Semilunarklappe aus diesem Gefäß abgehen. Diese Truncusklappe ist meist dreiseglig, sonst vier- oder selten zweiseglig an gelegt. Der Truncusursprung überreitet einen in der Regel großen VSD [17]. Bedingt durch den noch erhöhten pulmonalarteriellen Widerstand in der Neugeborenenperiode, werden viele Kinder erst in der 4. bis 6. Lebenswoche klinisch auffällig. Dann entsteht durch die natürliche Verminderung des pulmonalen Gefäßwiderstandes eine Lungenüberflutung mit konsekutiver Herzinsuffizienz. Eine evtl. vorher bestehende leichte Zyanose verschwindet. Tritt bei fortbestehender Zyanose eine Herzinsuffizienz bereits unmittelbar nach der Geburt auf, liegt in der Regel eine Insuffizienz der Truncusklappe vor.
