Zusammenfassung
Im Jahr 1926 beschrieb E. A. von Willebrand an einem Kollektiv von 62 Einwohnern der finnischen Aland-Inseln eine bis dahin noch nicht bekannte Form einer angeborenen Blutungsneigung, die er aufgrund des autosomalen Erbgangs, der verlängerten Blütungszeit und der bevorzugt auftretenden Blutungslokalisationen von der bekannten Hämophilie abgrenzen konnte. Die von ihm gewählte Krankheitsbezeichnung der Pseudohämophilie hat sich im internationalen Schrifttum nicht durchgesetzt. Heute wird diese hämorrhagische Diathese nach ihrem Erstbeschreiber als Von-Willebrand-Erkrankung (VWE) bezeichnet. Ausgehend von der Beobachtung einer verlängerten Blutungszeit bei gleichzeitig normaler Thrombozytenanzahl vermutete bereits von Willebrand eine Störung der Thrombozytenfunktion als zugrundeliegende Ursache der Blutungssymptomatik. In Untersuchungen mit einem selbst konstruierten „Thrombometer“ konnte von Willebrand zeigen, daß die Thrombusbildung im Blut von Patienten mit einer VWE im Vergleich mit gesunden Probanden deutlich langsamer erfolgte. In der Folge konnte bei VWE-Patienten eine Störung der Thrombozytenadhäsion, ausgelöst durch ein vermindert oder defekt synthetisiertes Plasmaprotein, nachgewiesen werden. Zusätzlich konnte gezeigt werden, daß bei der Mehrzahl der VWE-Patienten eine Verminderung der Faktor-VIII (FVIII)-Aktivität vorliegt. Dies und die Beobachtung, daß in biochemischen Reinigungsverfahren die Faktor-VIII-Aktivität zusammen mit den thrombozytenadhäsiven Eigenschaften des bei VWE-Patienten defekten Plasmaproteins isoliert wurde, führte zunächst zu der Vermutung eines identischen Proteins mit 2 unterschiedlichen funktionellen Eigenschaften. Auf diese Vermutung ist die lange Zeit übliche Bezeichnung des „FVIII-related antigens“ zurückzuführen. Erst mit verfeinerten biochemischen Methoden konnte gezeigt werden, daß der Faktor VIII und der Von-Willebrand-Faktor (VWF) tatsächlichzwei unterschiedliche Proteine darstellen, die im Plasma einen nichtkovalenten Komplex bilden. Nach Identifizierung des VWF-Gens und des VWF-Biosynthesewegs konnten wesentliche pathophysiologische Mechanismen der VWE aufgeklärt werden. Damit bildet die VWE ein typisches Beispiel, wie zunächst, ausgehend von einer klinischen Symptomatik, ein Protein und dessen funktionelle Eigenschaften identifiziert wurden und anschließend, basierend auf den naturwissenschaftlichen Befunden, eine exaktere Charakterisierung des zugrundeliegenden Krankheitsbildes möglich wurde.
