Zusammenfassung
Die Einführung einer neuen Methode wie die der Dopplersonographie in die Klinik läuft aus wissenschaftlicher Sicht optimalerweise in 3 Schritten ab:

Zunächst muß sichergestellt werden, daß eine klinisch interessierende Messung wie das Blutströmungsverhalten im fetomaternalen Gefäßsystem mit einer geeigneten Meßmethode (Dopplersonographie) überprüft werden kann (Fitzgerald u. Drumm 1977; McCallum et al. 1978).

In einem 2. Schritt sollte im Rahmen klinischer prospektiver Studien getestet werden, ob mit Hilfe dieses Meßverfahrens Schwangerschaften identifiziert werden können, bei denen aufgrund eines pathologischen Blutströmungsmusters ein höheres Gefährdungsmoment bezüglich Morbidität bzw. Mortalität zu erwarten ist.

In einem 3. Schritt wird dann anhand von vergleichenden randomisierten Untersuchungen geprüft ob die Überwachung von Schwangerschaften mit niedrigem bzw. hohem Risiko mit Hilfe der Dopplersonographie einen meßbaren Vorteil hinsichtlich Morbidität, Mortalität oder Kosteneinsparung nach sich zieht (Schneider 1993; Schneider et al. 1989, 1991).
