Zusammenfassung

Die Verwendung eines positiven endexspiratorischen Drucks (PEEP) erhöht

    die rechtsventrikuläre Nachlast (RV-Afterload)

    die rechtsatriale Vorlast (RA-Preload)

  5. 5.

    die funktionelle Residualkapazität (FRC)

Ein prophylaktischer niedriger PEEP (LO-PEEP) zwischen 1 und 5 cm H2O wird eingesetzt, um Atelektasen zu verhindern bzw. kollabierte Alveolen zu rekrutieren. Dadurch werden Ventilation-Perfusions-Ungleichmäßigkeiten vermieden, und die funktionelle Residualkapazität (FRC) wird oberhalb der Verschlußkapazität (CC) gehalten. Bei einer Erhöhung des endexspiratorischen Drucks um 1 cm H2O vergrößert sich die FRC um etwa 80 ml. Dieser niedrige PEEP hat für praktische Zwecke keine Nebenwirkungen.
