Zusammenfassung

Die Rolle des Alkohols für die Entstehung chronischer Nierenerkrankungen war bereits im 19. Jahrhundert bekannt, geriet jedoch in Vergessenheit und erregte erst in jüngster Zeit wieder größere Aufmerksamkeit. Alkoholkonsum führt zu morphologischen Veränderungen an der Niere, spezifisch im Rahmen des Syndroms der Alkoholembryopathie, subtil auch beim Erwachsenen mit Alkoholismus. Einige renal relevante Parameter sind eindeutig bei chronischem Alkoholismus verändert, z. B. kommt es zum gehäuften Auftreten von Hypertonie, Hyperurikämie mit chronischer Gicht, Elektrolytstörungen (die wichtigsten sind Hyponatriämie und Wasserintoxikation, Hypomagnesiämie, Hypokaliämie, Hypophosphatämie-Syndrom). Einem alkoholbedingten akuten Nierenversagen liegt am häufigsten Rhabdomyolyse, seltener Pankreatitis oder Hypophosphatämie zugrunde. Bei chronischem Alkoholismus sind generell alle Arten von chronischer Glomerulonephritis häufiger. Typisch ist die Häufung der mesangialen IgA-Glomerulonephritis sowie das häufigere Auftreten der postinfektiösen Glomerulonephritis mit ungünstigerer renaler Funktionsprognose.
