Abstract

The pilon fracture, a metaphyseal injury extending into the ankle joint, is difficult to treat successfully by any method. If the fracture into the ankle joint is displaced, the basic principles of open anatomical reduction and stable internal fixation, followed by early motion if technically possible, are valid. It is in this particular area, to an extent equaled perhaps only by fractures of the acetabulum, that the words “if technically possible” loom large; for this fracture is often so comminuted that technical difficulties cannot be overcome.
